When an individual submits a form in PCO, like the Dream Team Questionnaire or the Kids/Youth Ministry Application, the person is logging in when a name and email is provided. Planning Center is actually designed to remember logins for the user, so if a person is accessing a form on a cell phone, iPad, or computer previously logged into, PCO is likely to remember and log in when the form is accessed. This means that the individual's name and email is already entered at the top of the form. This is so helpful, unless a wife is logged in, and she completes a form for her husband, or a mom is logged in and completes the form for her son or daughter. Whoever is logged in is the owner of the form and the form will be attached to that person's profile in PCO.
To prevent this problem, the mom could log out first, and then start the form by entering the child's name and email, or the wife could log out and be sure to start the form with her husband's name and email. Another thing that helps prevent login confusion is to encourage spouses to use unique emails and phone numbers rather than sharing one for both of them.
So, you now have a DTQ that is on mom's profile, but is says the "Goes by name" is the child's name and the form is digitally signed by the child or on the wife's profile with the husband's name in "goes by" and the digital signature. (It also replaces Mom's or wife's birth date with the child's or husband's birth date and will alter the gender on her profile.)
There is no way to transfer a form that has been submitted from one person to another. There is no easy fix. There are two options: 1) You can tell the person about the problem and ask them to do the form again in the correct profile. Make sure they understand to start with the name and email of the correct person. 2) You can reenter the information for the individual and then submit a ticket to IT asking for the wrong form to be deleted. Option 2 is definitely the kinder choice, the opportunity for you to serve the individual.
Just go to the form. Pay attention to "My information" at the top of the form fields ({).
If you are logged in, be sure to use the icon in the top right corner to log out. If you get the request for your phone number to get a code, do not enter your number.
Just reenter the address or click the link again. Then you will have blank name and email fields to start the form as the correct person.
Have the form submitted incorrectly open in another window or print it for reference. Just retype each response exactly as it was submitted, and then submit when complete.
It is a good idea to check the profile of the person for whom you have submitted the new form, just to insure it has come through correctly. Once you have confirmed it exists on the correct profile now, please submit a Planning Center ticket to IT, requesting that the incorrect form be deleted. Be sure to include the name of the form and the name of the person who has the wrong form. If the wrong person was also added to workflows and needs to be removed, you can add that to the IT request.
